I realize that it is making many of you think of the book title, but I do want to point out that it is a family name.

We're aware of that, but it won't take out the book title reference from people's minds, because it's just too obvious.

Are actually going to know the middle name of the child? Does it matter if there's a book association with a middle name?

Nope - as I said in my post, I think the fact that it's a family name, and being used in the middle name spot means that it's fair game.

I was merely agreeing that just because it's a family name doesn't mean it doesn't also have an automatic association with the book, as well.
