Here's my thoughts:
Alex and Xela = Alex is a guy's fn to me, and I don't like that Xela is Alex backwards
Phineas and Sirius = Too out there for my liking
Narcissa and Lili-Rose = Narcissist? Too close for me. Lili-Rose is nice sounding and Lili is a nn for Elizabeth, but most people would think you were listing flowers.
Lafferty and Macaulay = Laffy Taffy? Macaulay Culkin? Don't like these.
My favorite twin names are (tentatively):
b/b James and John
g/g Katherine and Eleanor
g/b Sarah and Patrick
