Bettina is one of those nicknames that looks more like a full, legitimate name on its own (Lillian actually started out as a nickname for Elizabeth, but is used as a name by itself now). I don't think I'd ever use it, but it's all right. Personally I'd prefer a "Bette" nickname (pronounced like Midler, not Davis), but a real-life Bettina would probably end up going by Tina.
I agree. I think Bettina is better as a full name.

It's okay; I would use nickname Betty for it.
I think it should only be used as a nickname for Elizabeth and even then it's a bit precious. In the "Betsy" books by Maud Hart Lovelace, the ones when Betsy is a teenager, not a child, some character calls Betsy Bettina rather than Betsy, but I can't remember who it was. Her friend-later-husband-Joe? Or was it Tacy?
