I'm not a big fan. I do usually like word names, but Blaze is just a little tacky, I guess. Unlike most of the other commenters, I do prefer it to the spelling Blaise. "Blaise" sounds sickly, like a mashup of "chillblains" and "malaise."
Edit to add: I think the reason it comes across as slightly tacky is that I mentally group it with names like Rebel and Maverick; they all sound like names you'd give your son if you hoped he'd be a macho bad-boy action hero.

I think it's silly. There's a male Blaise who's an attorney in my rather large company, so it of course hasn't held him back professionally, but it still doesn't sound professional at all. I personally think it's better suited to a dog or a horse.
Not really a fan. It's one of those word names that don't seem substantial enough as a name to me.
GP only, maybe a MN. I like it in itself, but it's hard to take seriously IRL.
