Name of the child: Kalliope Lane (father's name is Kaleo and mother's name is Lani)
Country the child was born in: USA, Hawaii
Reason for being put up for adoption: Father died right after conception in the line of duty and mother died three days after giving birth due to unforseen medical problems. She did get to name her daughter beforehand, who is named after her father and mother.
