Not really fond of it for a female, may be because I'm a Dane and I'm used to it on males only here.
I can't say for other countries, mainly Scandinavian ones, where Kaj is a name, but in Denmark it is pronounced as KAI. I'd say though, that it could be as KAI in Norway too, since some names are pronounced similarly, given the close bond of both the Danish and Norwegian languages. (Norway was once a part of Denmark, for over 100 years ago. Or so.)
