I think it's possible in the UK, but only if you're a member of the nobility (!) and it's your title, not your actual given name. You could have been named Frederick George Augustus, but if your title is the Duke or Earl or Marquess or Whatever of Carlisle, then your friends and family would call you Carlisle. Like Prince Charles: no doubt some people do call him Charles, but his personal friends apparently call him Wales, which sounds depressing;y petulant to those of us outside the charmed circle!
