Kathleen is the accepted American spelling - Cathleen just looks wrong to me.

Lily Cathleen doesn't flow to me, I'm not sure why. Emma Kathleen / Cathleen is pretty enough. Lilia Cathleen / Kathleen is lovely, especially with the nn Lily. And it's no more unusual than Ginevra.
