Prior to Orphan Black I was saying it KOH-sih-mah, but after watching Orphan Black I now say it kuh-SEE-mah, like they do on the show, and I find I like it better that way anyway.
I do think it's usable in the US, though I'm sure it'll get all kinda of mispronunciations. Not that "normal" names don't get those too though tbh
