I can see it as a thin guy's nickname or a fat guy's if their trying to be mean.

Definitely 100% not as a given name. Even as a nickname it’s a little weird. Nobody likes to be defined by their weight. Especially since most people gain weight as they age, so even if the nickname didn’t start out mean-spirited, it will end up that way.
